The campaign in Call of Duty: Modern Warfare 4 hides its collectibles across three missions, not the whole story. Clear all of them and you walk away with four trophies: No Stone Unturned, Time to Crate, Noteworthy, and Never Forgotten. Most of the work lives in the open-world mission Stranded, which alone holds 26 Yellow Supply Caches and 58 notes.

Quick answer: Grab all four notes and four contraband in “Following Orders” for No Stone Unturned, read 50 of the 58 notes and open all 26 caches in “Stranded” for Noteworthy and Time to Crate, then open safe code 8410 in “Escape” for Never Forgotten.

Mission 3 “Following Orders”: No Stone Unturned

As a North Korean soldier, you search houses for defectors. The required items are spread across three houses; collect four contraband items and four notes to earn the achievement.

Inside the first home, the contraband is spread across three rooms. The MP3 player is in the bathroom medicine cabinet. A Western novel sits on the upper shelf behind the sliding doors in the bedroom. Extra rations are inside the upper white cabinet in the kitchen.

Note 1 is in the cabinet next to the rations, and it doubles as the safe clue. The recipe lists 2kg rice flour, 5g salt, 1L water, and 4 packs of raisins, then tells you to “skip the salt.” Drop the salt value and the toolbox code is 214.

Open the bedroom closet and enter 214 into the red toolbox. That gives you the heart medicine (the fourth contraband) and Note 2.
Move through the second home into the third. Note 3 waits in the wardrobe in the right-side bedroom.
Open the textured glass cabinet in the living-room bedroom to find the hidden cellar key, then use it on the kitchen floor hatch.
When the woman appears, choose “Please, come outside with me.” followed by “No. We’re going outside.” Drop into the cellar hiding spot and read Note 4 on the shelf. No Stone Unturned unlocks the moment you read it.

Mission 10 “Stranded”: Time to Crate and Noteworthy

Stranded is the free-roam mission as Pvt. Park Hyun-Suk. When you leave base camp with Kal, pick “I’ll find my own path and look for gear and supplies” so you can explore. Time to Crate needs all 26 Yellow Supply Caches; Noteworthy needs any 50 of the 58 notes read.

Every keypad code and how it is derived

Eight containers and bunkers are locked behind keypads. Each nearby clipboard lists “unintentionally used codes,” and the real code follows the pattern those entries set. The values below are fixed, so you can skip the math if you want.

One note at the SAM site writes three of the codes down for you: farm house 0330, river island 6789, and west island 0008.

Base camp, radio tower, and water treatment

Start at the cargo container by the radio tower. The clipboard on it is Note 1 and the keypad opens with 0024. Grab the supplies and the tactical map inside.

Climb the blue radio tower using the ladder, ledges, and pipes. Note 2 is on the intermediate platform. Keep going to the summit for Note 3 and Yellow Supply Cache 1, which holds the WZ.55 Striga Rampage.

Parachute southeast to the blue-and-white smuggler house and pick up Note 4 from the workbench. Next, visit the nearby shed with the orange door for Note 5. Continue east to the question-mark-marked water treatment building to find Note 6. Go through the floor grate for Note 7, then use the sewer board’s arrow sequence (right, left, straight, left, straight, left) to get Note 8 and Yellow Supply Cache 2.

On the sewer puzzle, follow the shape of the arrows on the board, not literal compass directions.

Checkpoints, the island bunker, and the southern monument

Clear Infantry Checkpoint #1 southwest of water treatment for Yellow Supply Cache 3 and Note 9. Cross the river west to the concrete island bunker, enter 0008, and collect Notes 10 and 11 along with Yellow Supply Cache 4.

At Logistics Station #200, south of you, take Yellow Supply Cache 5 from the green garage and collect Note 12. Head across the river to the north and scale the giant statue monument using its red grips to reach Note 13 at the top. Parachute northeast to the area beneath the bridge for Note 14, then secure Artillery Battery #7 to get Yellow Supply Cache 6.

Southeast village and the train line

Move southeast for Note 15 in a white building with a blue door and Note 16 on the teacher’s desk in the schoolhouse. At the southeast village container, read Note 17, enter 0078, and open Yellow Supply Cache 7.

Climb the water tower pole across the road for Yellow Supply Cache 8 on the gantry. Wait by the tracks, board the moving boxcar, and loot Yellow Supply Cache 9. Clear the main bunker tunnel entrance for Yellow Supply Cache 10.

Inside the bunker, follow Kal through the crowbarred door for Note 18, push the yellow cart to climb for Note 19, head upstairs for Note 20, and swim the flooded tunnel into the supply room for Note 21. Northeast of the bunker, two stationary red boxcars hold Note 22 and Note 23.

The northern stations and Jamming Station

Work north along the road: Logistics Station #10 holds Yellow Supply Cache 11, Checkpoint #2 holds Yellow Supply Cache 12, and Station #3 holds Yellow Supply Cache 13. On the snowy cliff path toward the Jamming Station, Note 24 sits by a fallen soldier.

The Jamming Station is dense with notes. The blue quarters building has Note 25, the outdoor helipad has Note 26, the main operations building has Note 27, the control room has Note 28, and the upper hallway has Note 29. Step onto the external tower base for Yellow Supply Cache 14. Logistics Station #20 to the west adds Yellow Supply Cache 15 and Note 30.

Drive southeast to the container, enter 8609 from Note 31, and open Yellow Supply Cache 16. The container by the water silo holds Note 32, and the northeast statue hides Note 33 on the corpse at the pedestal.

Back toward base camp and the SAM site

Clear the police-besieged farmhouse southwest for Note 34, then the bombed orange house (enter through the roof crash) for Note 35 and Yellow Supply Cache 17. North of base camp, use 0600 at the cliff container for Yellow Supply Cache 18. Note 37 is in a stone shed northwest of base, and Note 38 is across the river in a riverside shack.

At the highway sign, read Notes 39 and 40 on the container, enter 1234 for Yellow Supply Cache 19, and climb the pylon for Note 41. Blow up the barrier crates at Artillery Battery #6 with a drone for Yellow Supply Cache 20. In the northeast farm fields, use 0330 at the bunker hatch for Note 42 and Note 43. The river island container opens with 6789 for Note 44 and Yellow Supply Cache 21.

At Logistics Station #100, collect Note 45 by the broken elevator and Note 46 inside the guardhouse; these take you beyond 50 notes and unlock Noteworthy. Climb the elevator scaffolding to reach Yellow Supply Cache 22. Pick up Notes 47 and 48 in the abandoned house to the northwest. Stop the patrolling truck near Checkpoint 3 for Yellow Supply Cache 23, then breach Checkpoint #5 to claim Yellow Supply Cache 24.

Along the northern road, Note 49 is on a farmhouse floor. The booby-trapped roadside store needs a bomb drone to clear the tripwires; inside are Note 50 and Yellow Supply Cache 25. Finally, the SAM site holds Note 51 and Note 52, and its summit holds the 26th Yellow Supply Cache, which unlocks Time to Crate, with Note 53 behind it.

Notes 54 through 58 are not scattered in the world. Fast travel to base camp and repair the Grenade Launcher, Tactical Gloves, Tactical Boots, Snow Ghillie Suit, and Silenced Thermal Sniper at the workbench; each repair counts as a note.

Mission “Escape”: Never Forgotten and Gaz’s Dogtags

The prison escape mission has two notes and a safe. Find Note 1, the Confiscation Protocol Reminder, on a desk in the office opposite Cell 40. For Note 2, the Updated Code, go through the guard checkpoint to the security office; the code is 8410.

Use 8410 on the wall safe in the barracks office to get Gaz’s Dogtags. Opening the safe unlocks Never Forgotten.

Confirming each achievement

Each trophy fires the instant its condition is met, so you do not need to finish the mission to confirm it. No Stone Unturned pops when you read Note 4 in the cellar. Noteworthy pops after your 50th note in Stranded, and Time to Crate pops when you open the 26th cache. Never Forgotten pops as the barracks safe opens. If one has not triggered, you are still missing an item in that mission’s count.
