The Scanner is the standout addition. Point it at a patient at the front desk to get an instant read. Green means safe, red confirms an anomaly, and yellow means no target. It runs on a 120-second cooldown, so it replaces the slower photo and CCTV checks when you use it at the right moment.

Level 3 is where the class pays for itself. Every anomaly you shoot restores 4 sanity and gives 5 cash, so combat turns into a resource engine. That makes you nearly independent of sanity-recovery items in the late game when the hospital fills with threats.

With a team, Secret Agent pulls slightly ahead and carries runs a little deeper into the late game. It is a real advantage, but not one that justifies paying four times more for most groups. Both classes struggle with sanity management, since neither has a base regen like the Doctor or Surgeon.


How to get the most out of Secret Agent

No other class deals with high-threat anomalies as decisively as Secret Agent, so your job is to lean into that role instead of trying to do everything.

  • Play with a team. You get the best value alongside a Head Nurse or Surgeon who handles patients while you focus on threats.
  • Actively hunt anomalies while teammates treat patients. The Gun clears almost every anomaly, and at Level 3 each kill returns sanity and cash.
  • Save the Scanner for Special Shifts like Ambulance, where many patients are rushed in without checks. The roughly three-minute cooldown means you should not waste it.
  • Manage ammo. Spend bullets on hostile Skinwalkers and Bed Monsters rather than low-tier visual anomalies.

Note: The best return on the class comes at Level 3, where +4 sanity and +5 cash per kill make you nearly self-sufficient once the hospital is crowded with anomalies.


How to get the most out of Head Nurse

Head Nurse rewards a treatment-first playstyle. More slots mean fewer resupply runs and the ability to carry a full mix of healing items at once.

  • Fill your six-item inventory with healing items so you can treat patients quickly without constant supply runs.
  • Take charge of treatment and leave anomalies and check-ins to teammates with classes suited to those jobs.
  • Carry Coffee or Chocolate Bars at all times to cover the class’s lack of sanity regen.

Tip: If you cannot afford Head Nurse, buy the Nurse class for a small Animal Coin cost and level it to 3. A max-level Nurse with the Special Technique upgrade from the Supply Shop matches a max-level Head Nurse.


Free-to-play alternatives

You do not have to spend Robux to cover either role. Security starts with an X-Taser and Firefighter starts with a Fire Extinguisher, so both stand in for Secret Agent, though neither weapon clears every anomaly the way the Gun does. For Head Nurse, the regular Nurse class plus the Special Technique upgrade fully replaces it. If you want a strong free pick overall, saving up 2,500 Animal Coins for Surgeon is widely considered the best value.

The math comes down to how you play. Head Nurse gives you value on every single shift for a fraction of the price, which makes it the safer purchase for beginners and solo players.
