A few minutes after you hide Barney, an officer steps up to the check-in window. He introduces himself as Officer Duckman and says he is hunting a serial killer. When he shows the killer’s photo, the face is Barney’s. You then have two options.

If you turn Barney in, you can still finish the shift. Treat the wounded Officer Duckman by taking him to Room 7 and giving him the correct healing items.

How to confirm the Coffee Machine reward
You know the hide ending worked when Barney walks over to the booth after the officer leaves and hands you his suitcase. Inside is a second Coffee Machine, which brews faster than your starting one and speeds up Sanity recovery on long shift marathons. After this, Barney leaves the hospital and never returns.
Note: The reward only comes from staying silent. Holding E to alert Officer Duckman ends the storyline without the Coffee Machine, so wait out the full 30 seconds if the upgrade is your goal.
How to skip Barney entirely
Barney’s questline is optional. Close the shutter of your check-in window while his offer is on screen, and he leaves like any other desk interaction. You can do this at any point, including when his dialogue timer is running.
Rejecting him carries no penalty. You lose the police choice, the Surgery detour, and the Coffee Machine, but you also keep your full attention on photo checks, CCTV checks, and anomaly rejection. On busy nights with heavy anomaly traffic, turning Barney away keeps the desk clear.
Because Barney returns on later shifts, asking for coffee as if nothing happened, you can wait for a quieter night before committing to the full chain. Stock coffee in advance, then run him through all five requests when the rest of the shift is calm enough to spare the time.
