Partners join as you advance the main story. Each one carries its own hidden achievement, so working through the campaign in order handles the whole group. There is nothing extra to hunt for here beyond reaching the point where each character becomes available.

Collect a variety of each weapon and armor type
Each collection award asks you to log several different pieces of a given category into your Database. Weapons and armor count whether you crafted, bought, or looted them, so keep everything you find and let the Database fill in. Spread your loot across the towns and dungeons to pick up the rarer variants.

Six achievements ask you to learn every Sword Skill for a specific weapon type, which effectively means practicing with all of them across your playthrough. A seventh, Battle Master, only needs one single skill pushed to its maximum level, so it usually pops well before the full mastery set.

Fill out the Database
Your Database logs items, monsters, equipment, characters, and world entries the moment you interact with each for the first time. Talk to every NPC, fight everything, and open every container to grow it. Association Archivist is the early milestone at 10 world entries, and the rest want a variety across their categories.

A handful of achievements cover the crafting systems and the appearance menu. Each triggers on your first use of the relevant feature, so none of them require repeated grinding.

Arks are hidden off the main path, and clearing them opens new areas and shortcuts.

Note: Side quests unlock as you progress the main story and assemble your party. They tend to be tougher than the main missions, so keep your Sword Skills leveled or you may find yourself underleveled going in.

Nameless Hero and confirming completion
Nameless Hero is the platinum, and it unlocks automatically once every other achievement is earned. You do not need to do anything special for it beyond finishing the other 51. When each individual task triggers, the achievement pops on screen and appears in your PlayStation trophy list or Xbox achievement list.
If a collection or Database award has not fired yet, the usual cause is simply not hitting the required variety count. Keep logging new weapons, armor, monsters, and entries rather than repeating the same ones, since duplicates do not push those counters forward.
