Head and Back are the two gear slots in Grand Blue that stack stat buffs onto your character. You can wear one Head piece and one Back piece at the same time, layering bonuses like Health, Strength, Agility, and Precision on top of whatever outfit and accessories you already have equipped. Most pieces come from mob, mini-boss, and boss drops, while a handful are sold by the Smuggler or pulled from Treasure Maps and quests.
Quick answer: There are 10 Head pieces and 3 Back pieces. Farm them from enemy and boss drops, buy the common ones from the Smuggler, or earn them through Treasure Maps and quests, then equip them in the Head and Back slots.

How to equip Head and Back gear
Both slots sit under the Cosmetics section of your character menu. Drop a Head item into the Head slot and a Back item into the Back slot, and their buffs apply instantly. You can confirm it worked by opening your stats panel, where the numbers rise by the exact amounts listed for each piece. Since the two slots are independent, you can mix any Head piece with any Back piece to shape a build.
The Smuggler stocks several of the more common drops, so if a low-percentage grind is not going your way, check whether the item can simply be purchased instead.

The Head slot has the widest selection, covering everything from pure Health caps to fishing-focused gear. The table lists each piece, its stat buffs, and where it comes from.

| Terry’s Hat | +5 Fishing Luck, +10% Fishing Bite Speed | Complete the Tw-Worm Terry questline at Anchor Town Beach |

| Scrap Metal Jaw | +6 Health, +8 Strength | Axe-Hand Logan (20%, Anchor Town); Smuggler |

Head pieces with passive effects
Three Head items add a passive on top of their stat lines. These effects only work while the piece is equipped.
- Hypnotist Fedora — Master Hypnotist: your Berserk Hypnosis and Sleep Hypnosis last 50% longer.
- Kuro’s Spectacles — Silent Stride: +1.5 Walk Speed.
- Choppy’s Hat — +5% Chop Skill Damage.

All Back equipment stats and drops
The Back slot currently holds three capes and sacks. The two capes come only from World Boss Chests, which makes them the harder pieces to secure.
| Back piece | Stat buffs | How to get |
|---|---|---|
| Brown Sack | +13 Agility | Afuaru, the Hoarderer (5%); Smuggler (50 Gold) |
| Circus Leader’s Cape | +21 Health, +17 Agility | Choppy the Clown World Boss Chest (2%) |
| Marine Captain’s Cape | +26 Health, +12 Strength | Axe-Hand Logan World Boss Chest (2%) |
For a build focused on raw survivability, the Marine Captain’s Cape carries the highest Health value in the Back slot at +26 Health, while the Circus Leader’s Cape leans toward mobility with its Agility bonus. The Brown Sack is the easiest entry option, since it drops at a friendly 5% rate and can also be bought outright.
Once a piece is slotted in and your stat totals climb by the listed amounts, the gear is active. Rotate Head and Back pieces to match what you are doing, whether that means chasing damage with Bear Head and the Marine Captain’s Cape or setting up utility runs with Terry’s Hat.
