The Giant Piranha is the boss that gates your progress off the second island in How to Fish. It closes distance fast, hits hard, and constantly spits out smaller piranhas that crowd the arena. Beating it is only half the task, because the boss drops a skeleton that must go back to the lakeside NPC before the island actually opens up.

Quick answer: Give the woman by the lake three leeches to get the Modified Leech bait, cast it from shore to summon the Giant Piranha, then keep a shotgun on the boss while punching the smaller piranhas. After the kill, pick up the Giant Piranha Skeleton and return it to her to receive the coordinates for the third island.

What to bring before you cast the bait

Once you reel the boss in, there is almost no time to prepare, so sort your loadout first. The NPC even warns you to bring a gun, since fighting the Giant Piranha with bare hands is a losing trade. Eat to full health, equip your weapon, and leave one inventory slot open so you can pick up the skeleton afterward.

Skip the dynamite as your main combat tool here. It is cheap and useful elsewhere, but the self-damage makes it a risky way to clear the swarm. If you do use it, light and throw one only when the boss is grouped up with its minions, then get clear.

How to summon the Giant Piranha

Talk to the woman in blue overalls near the lake on the second island. She explains that she wants to cook her father something special and needs a big fish that loves leeches.
Search the ground around the island for leeches crawling in the grass. Walk up to one, wait for the pickup prompt, and grab it. Collect three in total. These are ground pickups, not something you catch with the rod.
Hand all three leeches to the woman. She turns them into the Modified Leech, the bait needed to trigger the boss.
Attach the Modified Leech to your fishing rod and cast it into the lake from shore. Reeling it in drags the Giant Piranha onto the island and starts the fight immediately.

How to beat the Giant Piranha

Unlike the first boss, the Giant Piranha never stuns, so there is no free damage window where it locks up. You have to keep moving and lead it around while firing whenever you get a clean shot. Treat it like kiting a fast enemy rather than trading blows.

When it lunges or bites, step sideways instead of backpedaling in a straight line. Running straight back lets the boss and its minions box you in against the water. The shotgun does the most damage when the piranha is close, but do not stand still lining up the perfect shot. Create space to reload, then return your fire to the boss.

The trees on the island make good cover. Stand next to one, and when the boss charges, loop around the trunk and shoot it as it overshoots. The AI is easy to bait this way, letting you circle safely while chipping its health down. A common alternative is to plant yourself on one edge of the island, run to the opposite side when it charges, and pick off minions from range before forcing a clean one-on-one.

Dealing with the smaller piranhas

As the fight drags on, the boss vomits out smaller piranhas. Each one deals little damage alone, but a group can overwhelm you quickly. They are twitchy and awkward to hit with a gun, so brass knuckles or another melee option clears them faster and saves shotgun ammo.

You do not need to chase every minion across the island. Only clear the ones blocking your movement or your line of fire, then get back on the boss. A useful bonus is that you can eat the dead piranhas to top up health or hunger during the fight, turning the swarm into a supply of healing.


Watch the white escape timer

The thin white bar beneath the Giant Piranha’s health is its escape timer. If it runs out, the boss disappears, and you have to rebuild the bait and start over. Keep dealing damage whenever it is safe, but do not let the timer pressure you into standing still and trading bites. Steady, safe hits beat reckless ones that get you killed.

Return the skeleton to unlock Island 3

Killing the boss is the combat win, not the progression win. When it dies it drops the Giant Piranha Skeleton, and there may be ordinary meat chunks nearby that are not the quest item. Grab the skeleton first before touching anything else.

Carry the skeleton back to the woman by the lake and hand it in. She rewards you with the coordinates for the third island, delivered as a flash drive. You know it worked when the hand-in dialogue completes and the new coordinates register. Return to your boat and check the Radar for the fresh marker pointing you onward.

If the island still won’t unlock

When the boss is dead but nothing advances, the issue is almost always a missed step in the chain. Run through these checks in order.

If attempts keep failing, change one thing between tries rather than repeating the same setup. Bring more cooked fish if you starved, earn money for a stronger weapon if the health bar barely moved, and stop chasing minions if the crowd kept overwhelming you. Once the flash drive is in your inventory and the Radar points away from the second island, the Giant Piranha is fully behind you.
