Early in Silent Hill: Townfall, a white and green cooler marked “HUMAN ORGAN IN TRANSIT” sits on the kitchen table in Zoe’s house. It has two three-digit combination locks, one on each side, and you cannot force it. Inside is Zoe’s Lanyard, the access ID that gets you through the door of the Shorefront Family Clinic, so the story does not move until the box opens.
Quick answer: The combination is the ET number on the Organ Transplant Card. The first three digits go on the left lock, the last three on the right. Most playthroughs show 956 and 133.
Where the Human Organ Box is in St. Amelia
Zoe’s house sits west of the town square in the Shorefront District, reached through the foggy back alley off West Shore Road. Go up the short flight of stairs, through the front door, and turn into the kitchen on the ground floor.

The cooler is on the wooden table, stamped with “Handle with care,” “Avoid extreme temperatures,” and the reference UNC3573. Interacting with it lets you zoom in on either lock and turn the tumblers one digit at a time.

Zoe’s message gives the formula: take digits one through three and the final three from the relevant ET code to form the storage container code. You can find ET codes on the Organ Transplant Card; use the number printed on the right.

Not every playthrough shows the same digits. Combinations such as 067 with 213, and 067 with 244, have also turned up on the same cooler. If 956 and 133 do not budge the locks, open the Clues tab, look at your own Organ Transplant Card, and split the right-hand ET number yourself.

What you need before the locks will open
The game will not let you skip ahead, even if you already know the numbers. You have to collect the clue items and get the electricity back on in Zoe’s house first.


Read the Organ Transplant Card from the Clues section rather than trusting memory. The ET numbers sit side by side, and the left one is not the one the locks want.
Entering the code and confirming it worked
You know both codes landed when the prompt appears at all. Pick Yes and a short cutscene plays, showing the lid coming up and a lanyard being pulled out of the cooler. You then receive Zoe’s Lanyard, an Access ID for the Shorefront Family Clinic issued to Zoe Ellis, District Nurse, and your map updates with the clinic as the next objective. The lanyard also appears in your inventory with a Read option if you want to look at the card itself.
What Zoe’s Lanyard unlocks next
Take the lanyard to the clinic entrance and use it on the panel to the left of the door. Inside, the Z Ellis office at the end of the corridor holds a Scalpel in a green medical case, which opens the rusted doors with a hatch in the middle, including the pharmacy and the gate at St. Amelia Park.
Hold onto the lanyard after that. It is one of the three items the Clinic scales puzzle needs later, alongside the Prescription Dosage Error Note from the pharmacy locker and the Broken Stethoscope from Jess’s house.

Once the cooler is open, the kitchen has nothing else worth searching, and the alley route back toward the town square and the clinic is the fastest way forward.
