They lead to a wooden shack wedged between two Trask Industries containers, with a doorway lit by purple energy.
Inside the shack is a dark room with a swirling column of purple energy. Interacting with it produces the prompt explaining that discovering a Nightmare Door unlocks the trial tied to it, and the associated story objective at this point is to track the captured mutants.
Nothing here is missable. If you walk past the door, you can return through chapter select or replay the trial later from the Nightmare Cabin.

Ranking is based purely on elapsed time, which runs at the top of the screen from the moment the three-second countdown ends. The thresholds are fixed.

Higher ranks pay more Adaptation XP, so clearing this one cleanly helps you reach Adaptation Level 10 earlier. You only need Adamantium in a single trial across the whole game, so if this one refuses to cooperate, a bronze finish still counts toward beating every trial.
How to clear the trial in under 45 seconds
The trial drops you into a foggy forest with a single path running forward. All three snipers sit on raised rock platforms along that path, and their red laser sights are the easiest way to spot them through the fog. Only sniper kills advance the counter.




A clean Adamantium run lands close to the wire. A finish of 00:44:79 clears the threshold by a fraction of a second, so every skipped ground fight matters.
Rewards and how to confirm you got Adamantium
The results screen is the confirmation. It lists your Time and Best Time, stamps the rank you earned, then ticks the AP counter upward. A full-speed clear runs it all the way to 18 / 18 AP and adds +1 STRAND before offering Retry and Continue.

Every trial can be run again from the Nightmare Cabin once the campaign is over, which is the easier route if you want the medal with a fuller skill tree behind you. Interacting with the bed there gives you access to the trials without replaying chapters.
Discovering all 14 Nightmare Doors unlocks the Pathways of the Mind trophy, and clearing all 15 trials at bronze or better opens the hidden memory tied to Repressed Memories. Hunting the Hunter counts toward both totals whichever rank you finish it with, so the 45-second push is optional once you have the door itself logged.
