Hozuki Pouches are Musashi’s main consumables for staying alive and topping off resources during fights in Onimusha: Way of the Sword. Each pouch does something different, from instant healing to refilling Oni Power, and every one can be pushed to Level 3 once you have the right materials. Getting all five and fully upgrading them is also what unlocks the Medical Marvel trophy and achievement.
Quick answer: You receive the Default pouch automatically during the mission Patching up the Pouch. You then collect four more Oni Cloths across Kyoto — Regeneration in the southeast, Aggressor in the southwest, Resistance south of the Shijo Street Spirit Mirror, and Awakening northeast of the Yasaka Pagoda Spirit Mirror (available only after you defeat Shuten Doji). Bring each pouch plus the listed materials to Okuni to raise it to Level 3.

All five Hozuki Pouches and what they do
There are five pouch types. Three restore health and related recovery stats, and two are built around offensive resources like Oni Power. You can only equip one at a time, so pick based on the fight in front of you.

Note: The Default pouch is the safest choice while you learn the game, since the burst heal lands the moment you use it. Regeneration and Resistance reward players who avoid taking hits back to back, while Aggressor and Awakening suit an aggressive style built around Oni Armaments and Musashi’s awakened form.

Every additional pouch comes from an Oni Cloth hidden in a Genma stronghold. After you finish Patching up the Pouch, Okuni marks each pouch location on your map. Some are locked behind miasma barriers, so if you cannot reach one yet, keep progressing the main story. In almost every location, a second chest nearby holds a Skeleton Hozuki, which you will need for the top upgrade.
Regeneration Oni Cloth (Southeast Kyoto)
Head to the Pouch marker in southeastern Kyoto, near the Gojo Street (East) Spirit Mirror. You cannot reach the chest from the street. Enter the northern courtyard, clear the enemies, and push through to a small gap in the wall into a second courtyard.
Climb the ladder to the rooftops, cross the petrified bridge over the street, and drop down. Activate Oni Vision to reveal the hidden chest holding the Regeneration Oni Cloth. A second chest in the same area contains Skeleton Hozuki #1.
Aggressor Oni Cloth (Southwest Kyoto)
Go to the Pouch marker in southwestern Kyoto, near the Gojo Street (West) Spirit Mirror. Work around the southern side of the houses and climb through the attic of the southern home to reach the far side.
Use Oni Vision to find the spider strings on the large gate, kill the spiders to open it, then defeat the enemies inside. Head right to a crawlspace under the wall, go through, and use Oni Vision to reveal the chest with the Aggressor Oni Cloth. A chest in the nearby alcove holds Skeleton Hozuki #2.
Resistance Oni Cloth (South of Shijo Street Spirit Mirror)
From the Shijo Street Spirit Mirror in northwestern Kyoto, travel south to the Pouch marker. On the west side of the buildings you will find a house with a ladder. Climb up and use Oni Agility to sprint across the petrified structure into the courtyard.
Push through to the Malice Barrier, break it open, and use Oni Vision to reveal the chest containing the Resistance Oni Cloth. Skeleton Hozuki #3 sits in a chest in the same area by the barrier.
Awakening Oni Cloth (Northeast of Yasaka Pagoda Spirit Mirror)
The Awakening pouch only becomes available after you defeat Shuten Doji. Head to the Pouch marker northeast of the Yasaka Pagoda Spirit Mirror, kill the spider hand to open the Oni wall, then defeat Byakue and move forward.
In the next courtyard you will fight a mix of Genma while Special Form Hitotsume Gasa attack from the walls. Once it is clear, crawl through the scaffolding in the northwest, then use Oni Vision to reveal the chest with the Awakening Oni Cloth. Skeleton Hozuki #4 is in the northern portion of that same courtyard.

The fifth Skeleton Hozuki (Hidden Treasure No. 5)
Four Skeleton Hozuki sit next to the Oni Cloth chests above. The fifth is tied to the Hidden Treasure No. 5 Chance Encounter, which appears later in the game. Complete the In the Light of an Icy Moon story quest first so the encounter is available.

How to upgrade Hozuki Pouches with Okuni
Okuni handles every pouch enhancement, and upgrades only open up after you finish the main story quest Patching up the Pouch. Return to her whenever you have collected enough materials. Each pouch has three levels. Going from Level 1 to Level 2 needs standard crafting materials like cloth, leather, iron, or whetstone. Going from Level 2 to Level 3 always requires one Skeleton Hozuki on top of the standard materials, which is why you need all five.

Tip: You can also refill a pouch between mirrors by using Hozuki plants found in the world. The rare Sun Dried Hozuki restores charges mid-fight, so save it for the toughest bosses.
Unlocking the Medical Marvel trophy
The Medical Marvel trophy and achievement pops once all five pouches sit at Level 3. That means collecting all four extra Oni Cloths, all five Skeleton Hozuki, and every crafting material in the table above, then completing each upgrade at Okuni. You can free-roam and clean up collectibles right up to the point of no return before the final mission, The Clash At Rendaino, so keep a manual save at that point if you still have work to do.
