Palworld: How to Use the Pal Surgery Table for Implants and Gender Change

Unlock requirements, every implant effect, gold costs, and how to flip a Pal's gender with the Pal Reverser.

Unlock requirements, every implant effect, gold costs, and how to flip a Pal’s gender with the Pal Reverser.

Breeding a Pal over and over just to land the right passive skills is no longer the only path. The Pal Surgery Table lets you rewrite a Pal’s passives directly and even change its gender, so you can take a wild catch and turn it into a proper base worker, mount, or combat Pal in minutes.

Quick answer: Reach Player Level 36 to unlock the Pal Surgery Table (Technology tier 38, 2 Technology Points), craft it with 30 Refined Ingot and 20 High Quality Cloth, then interact with it while the target Pal is in your active party to inject an implant or use a Pal Reverser to switch its gender.

Unlock and build the Pal Surgery Table

The table sits at tier 38 in the Technology tree and becomes available once you hit Player Level 36. Unlocking it costs 2 Technology Points. Also called the Pal Operating Table, it has 5000 HP and only functions inside a base.

Crafting requires the following materials:

  • 30 Refined Ingot
  • 20 High Quality Cloth
  • 10 Corrosive Solvent

Once placed, the table does two jobs. It adds or overwrites passive skills using implants, and it flips a Pal’s gender when you feed it a Pal Reverser.

Implant a passive skill onto a Pal

Move the Pal you want to modify into your active party using the Palbox. The operating table only shows Pals currently in your party, not the full box.

Walk up to the table and press the interact prompt to open the Pal Operating Table menu, then select the target Pal.
Pick the implant you want from the Passive Skill Selection list, then confirm with “Edit.” If the Pal has fewer than four passives, the new one fills an empty slot. If all four slots are full, you choose an existing passive to overwrite.

Approve the gold cost when prompted and select “Yes.” The chosen passive is granted immediately and, if you overwrote a slot, the old skill is gone for good.

You know it worked when the new passive shows in the Pal’s skill list and its stat panel updates. There is no refund, so double-check the slot you are replacing before you confirm. A common cause of the operation not appearing is having the Pal in the box rather than the active party, which hides it from the menu entirely.

Implant costs by rarity

The gold you pay depends on the rarity of the passive being implanted. Rainbow-tier implants are not obtainable yet, so that cost remains unset.

Standard implants available at the table

These reusable implants can be applied straight from the operating table without buying anything extra, priced at either 10,000 or 50,000 gold each.

Disposable implants for stronger passives

Buy extra implants from bounty and arena merchants

Six of the strongest standard implants have to be unlocked through merchants before they appear at your table. Three come from the Bounty Merchant and three from the Arena Merchant, each paid in that vendor’s own currency.

The Bounty Merchant guards a shack near the Small Settlement fast travel point. Talk to the vigilante, choose “Exchange,” and spend Successful Bounty Tokens. Bounties are scattered across the map, and defeating one permanently marks that NPC’s location.

The Arena Merchant, dressed in red, stands at the Arena Entrance in the northern desert. Choose “Exchange” and pay with Battle Tickets.

Change a Pal’s gender with the Pal Reverser

Gender swapping is the table’s second function, and it needs the right consumable. The Pal Reverser is bought at the Pal Arena for 35 Battle Tickets. With one in your inventory, put the Pal in your active party, open the operating table, and select the gender-change option to flip it.

This matters most for breeding, since a Breeding Farm needs one male and one female of a compatible pairing. If your two best Pals share a gender, reversing one avoids re-hunting or re-breeding for a match. The change confirms when the Pal’s gender icon updates in its status panel.

The practical payoff is speed. A freshly caught Pal can become a 50% Work Speed base worker with Artisan, a fast traveler with Runner or Infinite Stamina, or a safe capture partner with Mercy Hit, all without touching a Breeding Farm. Keep a stock of gold and merchant currency on hand, and the operating table quickly becomes the fastest way to tune your roster exactly how you want it.
