Difficulty in Pass the Fear is a trade. Picking a higher tier before a run raises your chance at better loot, but it also stacks penalties that change what enemies you face and how many Chaos Battle Scars you must reveal. Every step up sharpens both sides of that deal.
Quick answer: There are six tiers — Journey, Common, Hard, Nightmare, Hell, and Reality. Only Journey is available at first. Clear at least two bosses to unlock the rest. As the tier rises, both the boon (loot quality) and the curse (Battle Scars and future-chapter enemies) get stronger. Common is the one neutral tier with no boon and no curse.
Every difficulty tier boon and curse
The table lists each tier with its boon and its matching curse. Boons scale loot quality upward. Curses add Chaos Battle Scars and pull in enemies from later chapters. Higher tiers push both further in the same run.

At the start of the game, Journey is the only tier you can select. To open the rest of the ladder, clear at least two bosses. Once that condition is met, Common, Hard, Nightmare, Hell, and Reality become available for future runs.
You know it worked when the higher tiers stop showing as locked on the difficulty selection screen and can be chosen before you depart.

What Journey and Common actually do
Journey is the low-risk option. You take 50% less damage, which keeps early runs survivable while you learn the loop. The cost is slower progression, since you earn 50% fewer Soul Gems and less squad EXP. It is the tier to pick when you want practice without heavy punishment.
Common sits in the middle with neither a boon nor a curse. Nothing is boosted and nothing is added against you. It is the baseline reference point between the safe start and the punishing upper tiers.
How boons and curses change your loot and enemies
From Hard upward, the boon is always the same type of reward. Your chance of pulling higher-quality items and loot goes up, and it keeps climbing with each tier until Reality gives the best odds in the game.
The curse is where the danger lives. Starting at Hard, the game forces you to reveal Chaos Battle Scars before you set out, and it seeds your run with enemies drawn from upcoming chapters. Nightmare doubles the Scars and pulls enemies from two chapters ahead. Hell asks for three Scars. Reality is the hardest, layering three Scars at the start, another Scar for every chapter you clear, and enemies from four chapters ahead.
Because both sides scale together, moving up a tier is only worth it if your build and hunter can survive the extra enemies and Battle Scars long enough to collect the better drops.
Picking the right tier for your run
Match the tier to your goal. Choose Journey to learn safely, Common for a clean baseline run, and Hard or above only when you want the loot-quality boost and can handle earlier, tougher enemies plus the Chaos Battle Scars they force on you. Your hunter matters here too, since each has its own strengths that make certain tiers easier to clear. Weigh the reward against how many Scars and future-chapter enemies you are willing to fight before you commit.
