Quick answer: Fragments make five accessories: Preservation Jar requires 2; Manor Medicine Pot takes 4; Guardian Idol uses 4; Remnant Spark Cube needs 2; Nuo Mask calls for 3. You form each accessory after collecting all fragments in its set.
How accessory fragments work
None of these five accessories turns up in one piece in Phantom Blade Zero. You find fragments, and the inventory lists them under “Accessory Fragments”. They sit among the game’s other accessories, but they only become usable once the set is complete.
Picking up the second Preservation Talisman fragment while holding the first uses up both halves at once (“PRESERVATION TALISMAN FRAGMENT II -1” and “PRESERVATION TALISMAN FRAGMENT I -1”). The Preservation Jar appears immediately, and the game asks “Equip the new Accessory now?” That prompt tells you the set is done.
The Medicine Pot, Guardian Idol and Nuo Mask fragments work the same way. The Remnant Spark Cube finishes differently: its two halves belong to the tale THE CUBE, and its final objective is “Speak with Myst to learn more about the cube”. The accessory card credits Myst with restoring it.

Preservation Jar

PRESERVATION TALISMAN FRAGMENT I
The upper half of a talisman cleanly severed by a sharp blade. Grease and blood still stain the cut edge.
Its former owner appears to have been a greedy merchant, always using cheap tricks to deceive his customers. The butcher merely gave him what he deserved.
Look on the second table at the Butcher camp that also holds the Crimson Forest Tunnel Crank. It’s about 115 m from the Outlaw Trap bell, at about the same height.
PRESERVATION TALISMAN FRAGMENT II
The lower half of the talisman. Its clean break bears witness to the swift, decisive stroke that severed it.
When reunited with its upper half, it reduces poison damage.
This one glints in the second Butcher yard, about 43 m from the Outlaw Trap bell and about 17 m lower.
Manor Medicine Pot
Effect: “Using Medicine has a 8% chance not to consume a use.” All four pieces of the Manor Medicine Pot are glinting pickups in Pangzhen, close to the Huang Manor Crossroads bell.
Medicine Pot Fragment: Lid
The lid of a medicine pot fired from coarse clay, shaped like a knotted root and rough to the touch.

The head of the Guardian Idol, its eyes still wide with fury. It once witnessed its worshippers seek the Way through martial discipline, only to watch one of them raise a stone against it with trembling hands.

A broken tenon piece covered in twisted dents and gouges, resembling the scraps left behind after something has been gnawed away.
The enemy on Meigu Lake that carries the cube drops it. There’s no bell distance for this one.
Remnant Spark Cube Fragment: Mortise
A metal fragment with strangely angled folds. When held in hand, a distant hum seems to echo.

Nuo Mask
Effect: summon-type Phantom Edges cost 1 less Remnant Spark, and your summons gain Berserk. The three Nuo Mask fragments are glinting pickups tied to the Mirage Estate.
Nuo Mask Fragment: Joy
Three thousand guests feast behind vermilion gates, hosts and guests alike wearing smiling masks. When the feast ends, they unmask in search of old friends, only to find they’ve forgotten their own true faces.
