A bacterial wound infection in Project Zomboid is survivable if you treat it early, but leaving it dirty will drain your health and pile on pain. The important thing to understand up front is that the “Infected” status on your Health panel refers to bacterial wound infection, not the zombie virus. Those are two separate problems with two very different outcomes.
Quick answer: Clean the wound with a water source, disinfect it with alcohol (medical or liquor such as whiskey, bourbon, or vodka) by right-clicking the item and choosing Disinfect, then apply a clean or sterilized bandage. Swap the bandage as soon as it gets dirty until the wound heals.
Treat a bacterial wound infection
The game models real medical practice, so the treatment order is clean, disinfect, then bandage. Getting all three steps done and keeping the dressing clean is what stops the health drain and speeds recovery.

Note: In multiplayer, you can perform the same clean, disinfect, and bandage steps on another player by opening their Health menu.
Sterilize bandages to heal faster
A plain clean bandage stops health loss, but a sterilized one heals better and lowers the chance of the wound getting infected in the first place. Sterilize bandages using liquor or rubbing alcohol before you apply them. Ripped sheets can be sterilized too, which makes them a cheap, reliable dressing to carry.
Recovery speed depends on your overall condition. Being well-fed speeds it up, while hunger, thirst, tiredness, or a cold slows it down. Keeping the wound disinfected and re-dressed remains the fastest path to a full heal.
Wound infection vs. the Knox virus
The game never tells you outright that you have caught the Knox virus. The “Infected” label always means a bacterial infection, which is 100% survivable. Zombification is a separate, untreatable condition, and the two should not be confused.

Signs you have the Knox virus
A bite is a guaranteed death sentence. Symptoms surface over time, and within 2-3 days your character dies and reanimates, attacking any human on sight. Any non-bite wound from a zombie carries a smaller chance of infection, and the more severe the wound, the higher that chance climbs.

Even without a clear label, you can read the virus through Moodles that appear a day or two after a bad encounter for no obvious reason. Watch for these together:
- Anxious
- Uncomfortable
- Hot
- Queasy
Because there is no cure in the base game, the best defense is to avoid getting hit at all. Keeping your distance and never letting zombies land a hit is far more reliable than trying to treat the virus after the fact.
What a wound infection does to you
Bacterial infections do not kill you outright, but they hurt your performance. The main effect is pain, and pain has real consequences. In pain, your character can trip while sprinting or fall while climbing obstacles.
Higher levels of pain also cut your movement speed, attack speed, and damage output. That combination makes fights and escapes far riskier, which is why disinfecting and re-dressing the wound quickly matters even though the infection itself is survivable. Keep the dressing clean, stay fed and rested, and the wound will heal on its own.
