The Bulwark Stone sits outside the Citadel of Penance in Mortal Shell 2, not in a boss room. It is a Support Tarstone. Kill an enemy while it is equipped and you get a short burst of damage reduction.
Quick answer: Start at the Citadel of Penance Beacon, open the southwestern wall gate with the Citadel Gate Lever, cross the southern stone bridge, defeat the four enemies on the span, and loot the stone sarcophagus at the far end.
Citadel of Penance Beacon location
Open the world map and find the Citadel of Penance. The Beacon inside that fortress is the cleanest place to begin. The stone is not in the inner halls. It sits on the southern edge of the complex, past the walls.
On that same map, select the circular marker under the castle. That pin lines up with the southern approach. The path you need drops away from those walls and onto a long stone span that later breaks off.
Open the southern wall gate
Leave the Beacon and cut toward the southwestern wall. A gated opening there is the only way onto the southern bridge. The gate stays shut until you use the Citadel Gate Lever.
Note: If that wall will not open, you still need the Citadel Gate Lever. Do not look for a second entrance on the north side of the walls. The bridge route starts at this gate.

You have the right pickup when the item appears as Bulwark Stone. It is a Support Tarstone and can be used with any weapon or sidearm. The card trigger is a kill, not a block or a charged attack.

You know the effect is active when a kill grants that short damage-reduction window. If nothing changes after a kill, the stone is not equipped. Once it is in your Tarstone list, you do not need to return to the bridge.
