Cosmic Disease is a status effect in Mortal Shell 2 that skips damage entirely and goes straight for your Shell. Only one enemy in the whole game applies it, and only one item can cure it, which is why most players don’t even learn it exists until the final fight goes sideways.
Quick answer: Cosmic Disease is inflicted only by Zmey, the Unbidden, the final dragon boss. When the meter fills, it kills your Shell instantly (or kills you outright in Harbinger form). The counter is the Seedbearer’s Scripture, made by offering the Thawed, Depleted, and Faded Glands at the Forgotten Tower during Night Mode.

What Cosmic Disease does and who inflicts it
Cosmic Disease comes from Zmey, the Unbidden, and nothing else in the game triggers it. During the fight, Zmey uses a cursed fire that deals no direct damage. Instead, being caught in it builds a separate Cosmic Disease meter that appears on screen.
Two of his attacks apply it. The first is a flame breath that he aims across the arena. The second is a set of fire rings that spread out when he plants his sword into the ground. Both feed the same meter, so you can stack it fast if you keep getting clipped.
What happens when the bar fills depends on your current form:

The Seedbearer’s Scripture is a Permanent Pickup item built specifically to deal with Cosmic Disease. Its active use cures the status and grants a burst of mitigation, while its passive slot lowers how fast the meter builds. Because it counters the final boss, it is worth having in your inventory before you ever walk into Zmey’s arena.
You can hold only one Seedbearer’s Scripture per playthrough. Reaching Tier 3 familiarity requires three of them, which means finishing three separate playthroughs.
Where to find all three Glands
The Scripture is crafted from three Glands, each hidden in a different region. None of them is optional, so collect all three before heading to the tower.

How to trade the Glands for the Scripture

You’ll know it worked when the screen briefly cuts to black, and the Seedbearer’s Scripture appears in your inventory. The old bird prisoner then vanishes, which ends the questline.
Common reasons the offering won’t trigger
- You’re at the tower during the day. The prompt only shows in Night Mode.
- You’re missing one of the three Glands. All of them must be in your inventory.
- You’re looking at ground level. The interaction is on the tower’s upper floor, up the staircase next to the corpse.
Scripture effects by tier
The Scripture scales with how many copies you’ve collected across playthroughs. Its active use cures Cosmic Disease outright, while the passive slot reduces incoming buildup at all times.

Note: a balance patch adjusted these numbers, and some builds report the active use curing Cosmic Disease with 50% mitigation for 30 seconds plus a 15% always-on passive. Exact values can shift by version, so treat your in-game item description as the final word.

Using it during the Zmey fight
Equip the Scripture in the active slot before the fight, not just the passive one, or you won’t be able to cleanse the status when the meter spikes. Save the active use for the moment Zmey actually lands a Cosmic Disease hit. Triggering it then cures the effect and sends out a wave of yellow energy that briefly staggers him, opening a free punish window.
One trick makes the item stretch further. Because you can swap the Scripture between the active and passive slots mid-fight, you can fire the active cure and then immediately move it into the passive slot to keep the mitigation running right away. The Scripture won’t make Zmey easy, but it turns his most punishing mechanic from an instant loss into something you can survive.
