Your starter in Dragon Quest Monsters: The Withered World is decided by two things: which heroine you play as, and how you answer three quick questions from Fluffy at the very start. Pick Bianca and you draw from one set of four monsters. Pick Nera and you draw from a different set of four. The three answers then point to a single creature within that set.

How the starter monster is chosen
At the opening, Fluffy asks you to pick a heroine and then runs a short personality test of three questions. Each question takes a yes or no. The combination of your heroine and your three answers maps to one specific starter, so the same set of answers gives a different monster depending on whether you chose Bianca or Nera.
The two heroines also lean toward different monster families in battle, which is worth keeping in mind when you settle on one. Nera favors Slime, Nature, and Material families, while Bianca leans into Dragons, Beasts, and the Undead.

If you get the wrong monster
The quiz can be retaken right away. If Fluffy hands you a starter you didn’t want, you can redo the personality test and answer again until the correct pattern lands the monster you’re after. Confirm the creature on screen before you accept it, since that is the point where the choice sticks.
Your starter is also not a permanent commitment to your roster. Early on you can recruit other monsters in battle with a Show of Force, so a starter you dislike can be replaced within the opening area. Whichever creature you begin with, the answer patterns above stay tied to the same eight results.
These pairings come from the demo build, and progress can carry into the full game once it releases, so the answer-to-monster mapping is expected to hold. If any of the eight combinations change after launch, only the specific answers would shift, not the underlying two-part system of heroine plus three questions.
