In Call of Duty: Modern Warfare 4, Escape is mission number 13. You control Captain John Price as he breaks out of a North Korean prison, and there’s one optional collectible hidden in the level. Gaz’s dog tags are locked inside a wall safe; retrieving them awards Never Forgotten. A note found near the start gives the safe’s code, though the safe doesn’t appear until much later. If you miss the note, you’ll get to the keypad without knowing the combination. Story spoilers for Escape follow.
Quick answer: Once you and Ghost have cleared the barracks, use the first right-hand door, enter 8410 on the wall safe’s keypad, and take Gaz’s dog tags. Never Forgotten unlocks immediately.

The trophy is all you get. Gaz’s dog tags don’t grant an in-game reward or cosmetic, and grabbing them doesn’t change anything else in the mission.

Escape has two notes, and together they make up the code half of the trophy. Only the second one actually lists the number. The first is worth grabbing anyway, because the safe’s keypad prompt tracks how many notes you’ve read.
Collect both notes during the stealth opening

Open the safe after the barracks fight

After finding both notes, look at the keypad display. It shows the code alongside “Notes Found 2/2”, verifying that you collected the Updated Code note before you enter the numbers.

Confirming the Never Forgotten unlock
Never Forgotten pops the moment the dog tags are in your hands, with the description “Recover the Dog Tags in ‘Escape’.” You don’t need to finish the mission for it to count. Escape has no other hidden objectives after this, so head up to the garage control room and wrap up the level as usual.
If you left the safe unopened, you can still earn the trophy. Replay Escape whenever you like, on any difficulty. During the stealth section, read the Updated Code note, then retrace your route to the room beyond the barracks.
