Update 2 for Night At The Infirmary added a class system that changes how you survive the night shift. Each class hands you a set of passive perks, from a bigger starting Sanity meter to faster wheelchair movement, quicker operations, and stronger Sanity recovery. You buy classes with Shards, the currency you earn by treating patients and lasting through nights, and the two Legendary options can also be purchased with Robux.

Quick answer: Open the classes menu before you enter the portal, pick a class, then press the Shards or Robux button to buy it. There are nine classes across Common, Rare, Epic, and Legendary rarities. Surgeon (1000 Shards) is the strongest early pick, while Plague Doctor (6000 Shards or 360 Robux) is the best for solo runs.

Nine classes are currently available, sorted here by rarity. Rarity matters for two reasons. It sets the Shard price, and it controls how often the class shows up in the rotating shop. Legendary classes appear rarely, so you may see only one or none of them per rotation.

How to buy and level up a class

You handle everything from the lobby before you warp into the actual shift, so plan your purchase there rather than mid-run.

Press the classes button on the screen before you step into the portal that sends you to the main game.
Select the class you want and hit the Shards button to pay with Shards, or the Robux button for the Legendary options that support it. The class unlocks at Level 1 once the purchase goes through.

Play with the class equipped to earn Class EXP by completing tasks during a shift. Open the class in the menu and watch the EXP bar under its name to track progress toward Level 2 and Level 3.

Shards come only from playing. You bank them by treating patients correctly and surviving nights, so grinding a few full shifts is the fastest route to the more expensive Rare and Epic classes.


Class shop rotation and stock

The class shop restocks every 24 hours. When the daily timer resets, a fresh set of classes rotates into the shop. If the class you want is not on sale, the fix is simple. Wait for the next reset and check again.

Rarity decides how likely each class is to appear. Common and Rare classes show up often, while Legendary classes like Plague Doctor and Exorcist have an extremely low chance of stocking. Expect to see a single Legendary or none at all after a rotation, so buy one the moment it appears.


Best classes to buy first

Plague Doctor is the top choice for solo play. Because it is Legendary, you need to catch it in stock and pay 360 Robux or 6000 Shards. It reduces the effect of a drained Sanity meter, speeds up operations, and moves your wheelchair faster, so you can treat patients and clean beds quickly to recover Sanity. When patients turn into anomalies and trigger jumpscares, the reduced Sanity effect softens the damage.

Surgeon is the ideal starter class. At 1000 Shards it is affordable, and it stacks strong wheelchair and operating speed. By Level 3 its 83% operating speed boost makes it a standout pick for fast clears and speedruns.

One limitation to keep in mind. You cannot gift a class to a friend right now, so each player has to buy their own. If you run in a squad, the team-wide perks on Ward Assistant and Therapist are worth coordinating, since their Sanity and operating bonuses apply to everyone in the session.
