Challenge 4 in the Prologue of Halloween: The Game asks for one specific kill: “Throw an Orderly to his death.” It does not count a bare-hand execution, a wall slam, or the telephone pole. The game only credits it when you drag a living orderly to a ledge or railing and use the environmental prompt that appears there.
Quick answer: Grab an orderly, walk him to a railing or ledge until the skull icon appears, then hold the interact button (F on keyboard, shown as “Toss Over”) to throw him over. The banner reading “CHALLENGE 4 COMPLETE” confirms it, and you still have to finish the Prologue for that progress to save.
Where the railings and ledges are in Smith’s Grove
Smith’s Grove has more than one valid drop point, and the challenge does not care which you use. What matters is that the game registers a Context Execution at that spot, marked by a skull icon on your HUD. Three locations are reliable.

The power-box railing pairs well with Challenge 3, since the telephone pole sits on the right side of the same power unit. Do the pole execution first, then continue along the nearby path and take the right toward the throw point. There is an orderly farther along shining a flashlight at a patient who works as your target.

Why the throw fails, and what to do next
The most common failure is the message “ORDERLY ESCAPED” appearing mid-drag. Orderlies struggle while you carry them, and if the grab breaks before you reach the ledge, he runs and the attempt is over. That is not a run-ending problem — simply move on to the next orderly. A common recovery is grabbing a second target near a staircase railing further along the route.
- Nothing happens on the interact button: you are not fully inside the zone. Reposition until the skull icon shows.
- Michael drops the orderly: another orderly shoved you. Thin out the area first.
- You executed instead of tossing: a bare-hand or wall kill does not count toward this challenge.
Note: Challenge progress banks only when you finish the Prologue. If Challenge 4 registers but the rest of the run goes badly, escape anyway rather than restarting — quitting out throws away the completion you already earned.
Rewards for completing Prologue Challenge 4
Throwing an orderly to his death unlocks two profile cosmetics: the Groovy Style Profile Icon and the Borealis Profile Background. Both are equipped from the main menu by opening the Graveyard tab, selecting your Profile, and choosing Customize.
Where Challenge 4 fits in a full Prologue run
The six Prologue challenges pull in two directions. Five of them reward direct contact with the orderlies, while the no-alert challenge punishes it. Trying to satisfy everything in a single run generally costs more time than splitting it in two.

Sequencing matters more than it looks. Spend your early orderlies on bare-hand kills and you may reach the power box with nobody left to drag to the pole or the railing. Keep at least two alive as you approach that part of the grounds, take the pole kill first, then handle the throw on the path just beyond it. Once both register, you are free to sweep the rest of Smith’s Grove and head for the gate.
