Clive Barker’s Hellraiser: Revival puts a full trophy and achievement set behind its trip through the Labyrinth, and nearly all of it is reachable without replaying the campaign. The catch is that the game has no Chapter Select and no free roam once the story ends, so the optional trophies can be lost for good if you push past them.
Quick answer: All 45 console trophies, including the PS5 Platinum, can be unlocked in a single playthrough as long as you spread out your eight manual saves and reload whenever a missable objective slips. The Steam version adds 25 more achievements on top.

Trophy and achievement counts by platform
Consoles carry 44 trophies and achievements, plus the Platinum on PS5, for a total of 45. Xbox runs the same list minus the Platinum. The Steam release keeps the core set and bolts on 25 PC-only achievements tied to combat, crafting, and weapon handling.
Most story trophies unlock on their own as you progress. The risk sits with the optional combat, exploration, and shop objectives, several of which can be permanently missed.

Because there is no way to revisit chapters or roam after the credits, your manual-save slots are the only safety net. Stagger them across the campaign rather than overwriting one slot, and make a fresh save before any section that demands stealth, a one-shot setup, or a branching choice.
Reach the end of the story on a single save and you can lock yourself out of missed trophies with no recovery option. Keep a save in reserve before each tricky objective so a failed attempt only costs a reload.

The missable trophies that need planning
A Quiet Place (graveyard stealth)
For this graveyard route, go across and come back without being spotted or tossing anything. At the entrance, search the corpse for the Shotgun and ammo, then create a manual save beside the nearby wall rift.
Move strictly from bush to bush, keep crouched, and avoid the broken glass on the ground. Do not pick up stones or bottles, since using a throwable breaks the trophy condition. Squeeze through the hole in the brick wall to reach the upper section, where a single patroller is the only threat.
Inside the cabin, open the safe with code 9606 for a weapon part, take the Shed Key from the bedroom nightstand, and grab the Inventory Capacity Upgrade from the hallway closet. Use the key on the outside shed to collect the Graveyard Gate Inset, make a second manual save, then retrace the same foliage path back. Place the Inset into the angel statue relief at the main gate to unlock the trophy. Hug the eastern side and use cover to slip past Charlotte on the return.

What’re Ya Buyin? and What’re Ya Sellin?
Both shop trophies can stop tracking after the first store on PS5, so settle them at Zora’s first shop. Make a manual save nearby, then sell your items and spend your coins, repeating until you clear 666 spent and 999 earned. Reload the save and try again if either fails to pop.
Red Dead Damnation
Bring enough ammunition before heading to the docks shooting range, where you must hit all nine bottles. Make a manual save first so you can retry if you miss or run out of ammo.
Guns, Lots of Guns and Lord of War
For Guns, Lots of Guns, collect five firearms in this sequence: Pistol, Shotgun, SMG, Revolver, and Assault Rifle. To earn Lord of War efficiently, fully upgrade the SMG; most of its attachments can be found rather than bought.
Librarian and Artificer
Librarian wants all 12 crafting recipes collected. Artificer needs you to craft every available item at least once. The game does not reliably track which items you have already made, so keep your own checklist to be sure nothing is skipped.

Both endings in one playthrough
Experience Beyond Limits and Human After All come from two opposing final choices, accepting or denying the Cenobites’ offer. Make a manual save right before the closing decision, earn one ending, then reload that save and pick the other to collect both trophies without a second run.
Once every trophy registers, To Hell and Back unlocks automatically on PS5. If you kept your saves spaced out and handled the missable objectives as they came up, the whole set is achievable in that single playthrough.
