Quick answer: The Burial Grounds bell is a wooden cross beside a candle-lit gravestone shrine in Crimson Forest. From the Mirrorwood Lóng Pillar bell, go past a broken plank pile, grapple off a branch, and drop into a poison gorge.

Where the Burial Grounds bell is

Burial Grounds sits in Crimson Forest in Chapter 1 of Phantom Blade Zero, further along from the Mirrorwood Lóng Pillar bell. It is a misty clearing with bodies hanging from the trees and a stone arch at the back. The bell is a wooden cross standing on broken boards next to a large carved gravestone shrine. Red candles and a kneeling figure sit in front of the shrine.

How to reach the Burial Grounds bell from Mirrorwood Lóng Pillar

The route involves a long drop and a stretch of poison mist, so arrive with healing ready. On PATHBREAKER, the default difficulty, both cost a lot of health.

Start at the Mirrorwood Lóng Pillar bell, the wooden cross in front of the dragon statue. Cross the creek basin, about 40 metres, to the misty waterfall pool. The pool is on the opposite side from the opening you used to enter the basin.

Go up onto the rocky, wooded ledge next to the pool. From there you can see a pale stone path through the trees ahead.

Head toward that path until thick bushes stop you at a broken pile of old wooden planks. Walking or jumping at the planks gets you nowhere.

Go around the planks through the gap in the bushes on the side away from the waterfall, about 20 metres. A heap on the ground along the way can be picked up with E for 2,049 Remnant Dust.
Come out onto the pale stone stream bed, with a big mossy boulder nearby and a ruined tower on the cliffs beyond. Look up at the tree branch hanging over the stream bed until the “L Ctrl” grapple prompt appears on it.

Press Left Ctrl. The chain swings Soul out from the branch and drops him into a deep gorge with a pale stone road at the bottom. The fall takes most of a full health bar on PATHBREAKER, so heal once you land.
Follow the road along the gorge. The green mist hanging over it is poison and drains health while you stand in it, so keep moving and heal once you are clear.

Keep going for about 70 metres until the gorge opens into the Burial Grounds clearing. You will see the hanging bodies, the stone arch, and the gravestone shrine with red candles.

Walk up to the wooden cross beside the shrine. When the E prompt shows on Soul, press E to rest at the bell.

Resting also autosaves, and the save list then shows entries named “Burial Grounds”. Burial Grounds also becomes a Traverse destination, so you can come back without repeating the drop and the poison gorge.

Enemies past the Burial Grounds bell

About 45 to 60 metres past the bell, a pack waits together. It includes a Shield Brute, a Corrupted Brute with an axe, and two Corrupted Arsonists who throw fire bottles. On PATHBREAKER this group is a hard fight when it attacks at once.

The fire bottles keep burning you after they hit, so step off burning ground and heal early. The bell is close by, so rest at it before you engage the pack.
