The Records Room in St. Amelia Medical Centre is sealed behind an electronic keypad, and there is no key, no vent, and no second door. Four digits are the only way through, and the medication locked inside is required later, so the room is not optional.
Quick answer: At the keypad beside the Records Room doors, input 2605. The Office bulletin board one floor above has the note containing this code.
Records Room keypad location and what it accepts
The corridor includes Surgery, Chapman Ward, the Patient Rooms, and the Ward Office, with the Records Room along it as well. Its sign is immediately beside a pair of double doors, and a small keypad is attached to the adjacent wall. The keypad shows 1 through 0, along with X, Y, and Z, although this one accepts numbers only.

Four things matter for this lock, and knowing where each one lives saves a lot of flashlight-waving through identical hallways.
| Item | Where it is |
|---|---|
| Medical centre map | Wall left of the Operating Room, at the chapter’s start |
| Code note | Office, northwest corner beside the Private Ward |
| Keypad | Wall beside the Records Room double doors |
| Hydroxalene 3mg | Metal cabinet on the left inside the Records Room |
Grab the map if you walked past it. It labels the Records Room, the Office, and the Private Ward, and it later picks up handwritten dosage notes for the three drugs you are collecting.

The keypad accepts 2605 whether or not you have read the note, so the trip upstairs is skippable if all you want is the medication.
Where to find the Records Room code note in the Office
You can also make the brief trip to find the code yourself. Once the note is read, the digits are stored in your records, so there is no need to memorize them while returning downstairs.

Hydroxalene 3mg inside the Records Room
Past the doors, the room is rows of metal filing cabinets, cardboard boxes, and storage shelving. The one that counts is the tall metal cabinet on the left-hand side.

Opening the cabinet exposes a green first aid kit; open the kit as well to find a syringe marked Hydroxalene 3mg. Collecting it adds the dose to your inventory and confirms that this room is cleared.
One of the three listed medications is now accounted for. Fenzobromide and Dextrecyclate remain somewhere in the building; specifically, a Dextrecyclate 2.5mg syringe is suspended from an IV stand beside a hospital bed in the ward. The chapter’s final dosage puzzle requires all three syringes before it accepts an entry, so continue searching the rest of the medical centre.
