Hozuki Pouches are the healing and passive-buff slots in Onimusha: Way of the Sword, and they are also one of the longer completion chains in the game. There are nine pouch pickups scattered around Kyoto, split between four Oni Cloths and five Skeleton Hozuki, and each pouch has its own upgrade path with its own material bill.
Quick answer: Collect all nine pouch items (4 Oni Cloths, 5 Skeleton Hozuki) and raise every pouch to level 3 before you start the final mission, “The Clash At Rendaino.” The Medical Marvel trophy unlocks on the last upgrade.
What Hozuki Pouches do
Each pouch applies a passive boost while equipped. The default pouch increases your health. The others cover stamina, special attacks, and ultimate mode, so the one you run depends on whether you want survivability or output. Upgrading a pouch with supplies raises that same passive effect rather than changing it.
Five pouch variants exist in total: Default, Regeneration, Aggressor, Resistance, and Awakening. The Awakening pouch is gated behind story progress and only becomes available after you defeat Shuten Doji, so don’t hunt for it early.

The system opens up during the mission “Patching up the Pouch.” Speak with Okuni and she sends you out for the materials needed to push the pouch to its first upgrade level. From that point on, every pouch you own can be enhanced whenever you have the right supplies in your inventory.
Pouch pickups appear as their own markers on the world map, but the marker only tells you roughly where the chest is, not how to reach it. Most of them sit behind Oni Gates, Malice Barriers, or hidden Oni Spots that stay invisible until you activate Oni Vision. Several are guarded, and the enemies have to be cleared before the door or chest will open.

Tip: buy Oni Vision: Treasures, Oni Vision: Treasures 2, and Oni Vision: Search Range as early as you can afford them. Holding L2/LT and tapping R2/RT while you move keeps chests and hidden spots flagged, which cuts most of the guesswork out of pouch hunting.

Eight of the nine sit in pairs, which is the useful part. Every pouch marker area holds one Oni Cloth and one Skeleton Hozuki, so a single trip clears two items. The ninth comes from a Chance Encounter instead.

Skeleton Hozuki 5: The Hidden Treasure No. 5 encounter
The last Skeleton Hozuki isn’t tied to a pouch marker at all. It sits inside the Hidden Oni Treasure #5 chest, which you open during the “Hidden Treasure No. 5” Chance Encounter. If you are skipping side content, this is the one that quietly locks you out of the trophy.
Hozuki Pouch upgrade materials for every level
Every pouch starts at level 1 and caps at level 3. The four variant pouches begin at level 1 the moment you pick up their Oni Cloth, while the default pouch reaches level 1 automatically through “Patching up the Pouch.” Level 3 always costs one Skeleton Hozuki, which is why you need all five.

Two of those costs pull from limited stock. Fine Silk Cloth and Tamahagane are also used for clothing and weapon enhancements, so budget them before you dump everything into gear.
Confirming Medical Marvel unlocked
Medical Marvel pops the moment the last pouch hits level 3. Nothing else is required, so if you have applied every upgrade and the trophy hasn’t fired, one pouch is still short of maximum. Open the pouch list and check each entry individually rather than assuming the Awakening pouch is done, since it is the easiest one to leave at level 1 given its Shuten Doji requirement.
The usual reasons it doesn’t trigger are simple. You are missing one of the five Skeleton Hozuki, most often the Chance Encounter one. You never picked up the Awakening cloth because the Shuten Doji fight hadn’t happened yet. Or you spent your Fine Silk Cloth and Tamahagane on equipment and can’t cover the Default and Resistance upgrades.
Your deadline and what carries into New Game+
Free roam stays open right up until the point of no return prompt in front of “The Clash At Rendaino.” Everything above can still be collected at that stage, and if you keep a manual save there you can come back and clean up later. After the credits, choosing Continue in the main menu also drops you back before the final mission.
Hozuki Pouches and their enhancements carry over into New Game+, along with items, charms, skills, and awakened Oni abilities. That means a second playthrough on Carnage doesn’t undo any of this work, and if you’d rather finish the pouch hunt on the second run, nothing is lost by waiting.
The practical order is to gather all nine pickups first, then do your upgrading in one sitting once you can see the full material bill in front of you. Skeleton Hozuki are the bottleneck, and spending one on a pouch you don’t intend to finish is the only mistake here that costs you real time.
