Stamina is the thing that quietly decides most early fights in The Blood of Dawnwalker. Swing too often, block one hit too many, and Coen is standing flat-footed in front of something with claws. The Arbiter’s Medallion fixes a lot of that, and it is sitting in a cave you can reach in about two minutes of running.

Quick answer: Fast travel to the Sandy Gorge shrine, run northwest to the river bridge, drop down the cliff beside the waterfall, swim into the hidden cave behind the falling water, and loot the skeleton on the cave floor.


Arbiter’s Medallion stats

The Medallion is a Legendary Amulet listed at Level 9. Every bonus on it feeds into the same thing, which is letting you attack and block more often before the bar empties.

Because it gives nothing to Vampiric Health, this is a daytime, sword-and-shield pick rather than a night build item. The flavour text on it reads: “No one’s more worthy of wearing it than you.”

Note: Despite the name, the Medallion has nothing to do with the Arbiter Armor set, which is forged much later by the Uriash Hermit blacksmith during A Bulwark Against Darkness. You do not need that quest, or any quest, to pick this amulet up.


Where the Arbiter’s Medallion is

The amulet is in an unmarked cave hidden behind a waterfall, in the river basin under the bridge. On the map, that spot is north of Yewtree Outpost and northwest of the Sandy Gorge fast travel shrine. There is no quest marker and no icon for the cave, so the bridge is your landmark.


How to reach the waterfall cave from Sandy Gorge

Fast travel to the Sandy Gorge shrine, north of Yewtree Outpost. If you have not unlocked it yet, walk up from the outpost and activate the shrine with the on-screen prompt so you can return here later.

Open the map and drop a custom marker on the river directly under the bridge, northwest of the shrine. The cave has no icon of its own, so the marker is the only thing keeping you pointed at the right stretch of water.

Head northwest along the path until the river and the bridge come into view. Stay on the trail rather than cutting across the gorge, and you will end up above the waterfall.
Climb down the cliffside on the left of the waterfall to reach the water below. Take the ledges in short hops instead of one long fall, since fall damage this early can end the trip.

Swim toward the right-hand side of the waterfall and push through the falling water. The cave mouth is behind the curtain of water and is not visible until you are almost inside it.

Follow the cave in and look for the skeleton lying on the floor. Loot it, and the Arbiter’s Medallion is yours.

How to confirm you picked it up

Open your inventory and check the accessories tab. The entry should read Arbiter’s Medallion, Legendary Amulet, Level 9, with a weight of 0.20 and a value of 337. Equip it in the amulet slot and your maximum stamina figure should climb by 15% immediately.

If you came away empty handed, it is almost always one of two things. Either you swam to the wrong side and never found the opening, since the entrance is on the right of the falls, or you looted the chests and silverware but walked past the skeleton on the ground, which is the only container that holds the amulet.


Clear the rest of the cave for Denarii

Do not leave straight after the skeleton. The cave is packed with chests and loose valuables, most of it silverware and gold-plated tableware that exists purely to be sold to vendors.

None of it weighs much, and a full sweep is a solid Denarii payday at a point in the game where you have almost nothing to spend. Given how early the cave opens up and how cheap the detour is, the Medallion is worth grabbing on your first pass through the region rather than saving for later.
