Lacra is meant to gather information on Brencis, not fight him, but her history with him and Isbrand pushes her to act anyway. If you storm Greifberg Castle with her at your side and nothing has changed, Brencis kills her in the ensuing fight, and there is no in-the-moment choice that stops it. The decisions that keep her alive all happen well before that battle begins. Note that what follows touches on late-game events and endings in The Blood of Dawnwalker.
Quick answer: Finish the Night of Horrors quest to unlock the Mandrake Ward before you start the Midnight Reckoning quest, or defeat Brencis without partnering with Lacra at all. Either path lets her survive.

The two ways to save Lacra
Only two outcomes end with Lacra alive. You can protect her directly so she survives Brencis’s attack during the castle fight, or you can remove her from the equation by beating Brencis on your own. Everything else on the way to the Castle leaves her exposed.

Before the Midnight Reckoning quest to infiltrate the Castle, Lacra says she has an idea she wants to try first. That idea becomes the Night of Horrors quest, and completing it is what keeps her alive during the assault.
You know it worked when that same defeat cutscene ends with Lacra pulling through instead of dying. If you complete the quests in the wrong order, or run out of time segments and skip the Mandrake entirely, she will not survive the encounter.

Save Lacra by beating Brencis without her
If you never bring Lacra into the final fight, she cannot be killed in it. Several routes end with Brencis dealt with and Lacra still alive, though she simply flees afterward, annoyed that Coen handled him without her. There is no special send-off in these endings; she survives but stays on her own.
- Accept Brencis’s parley and defeat him there.
- Infiltrate Greifberg Castle and defeat him alone.
- Start the Rise at Dawn quest and defeat Brencis with Marat.
All three give Lacra the same conclusion, where she lives but continues her journey by herself.

The choice that gets Lacra killed
Siding with Brencis and keeping him alive removes both safety nets. In that case, Lacra attacks him on her own and dies in the attempt. Bringing her into the castle fight without the Mandrake Ward produces the same fatal outcome.
Absorbing the Mandrake’s power with her, or defeating Brencis without her, are the only ways to keep Lacra alive. Lock in one of those before the final confrontation and her story continues past the Castle.
