The Clown is a hidden NPC in Animal Hospital, added on September 5, 2026 alongside the second floor and its Laboratory. He sits in a sealed cell underneath the lab, and the only way in is a floor vent that most players walk straight past. Finding him takes about thirty seconds once the second floor is open, and it pays out a Journal achievement.

Quick answer: Reach Shift 6 to unlock the second floor, take the stairs opposite Room 6, walk to the lab corner beside the Insert Sample machine, stand on the floor vent and jump until it breaks, then drop in and speak to Squiggles.


Clown location and requirements in Animal Hospital

Nothing about the Clown is random. He is always in the same room, the room is always under the same corner of the lab, and the lab only exists on the second floor. That means the whole task is gated behind one thing: getting upstairs.

The second floor becomes available once you survive the emergency ambulance shift and roll into Shift 6. Keep your sanity up and reject anomalies as normal until then, because dying resets the run and you lose the trip upstairs. How to break the lab vent and reach the hidden cell

Play through your shifts until Shift 6 begins. Treat patients, reject anomalies at the desk, and use coffee or food from the shop to keep sanity from bottoming out.

Head to the stairs opposite Room 6 and go up. The upper level holds a break room and the Laboratory.
Walk into the lab and move to the back corner, right beside the Insert Sample machine. A metal grate is set into the floor there, easy to miss against the tiles.
Stand on the vent and jump on it. After a moment the grate gives way and swings open into the dark space below.
Drop through the gap. You land in a cramped room with a barred cell, a shelf, and a locker against the wall.

Note: nothing down here attacks you. Squiggles stays behind the bars for the entire conversation, so you can take your time reading the shelf.


Talking to Squiggles and freeing him from the cell

Interact with the figure in the cell to start his dialogue. He introduces himself as Squiggles, rambles about balloons, and promises to bring you some if you open the door. Most of what he says loops back to balloons, but the outline of his story is consistent with the paperwork stored a few feet away.

Behind the open locker in the same room there is a door. Opening it reveals a passage that connects straight back to the Reception office on the ground floor, which doubles as a shortcut for the rest of the run. Freeing Squiggles is a separate interaction, and once you do it he backs into the darkness and does not reappear.

Clown achievement reward and how to confirm it unlocked

Releasing him awards the “Clown” achievement, worth 10 Animal Coins, with the description “Release the clown. Honk Honk.” The achievement pops on screen during the shift, and you can check it later in the Journal from the menu before you enter a playthrough.

Two things in the room confirm it worked even if you missed the pop-up. The cage is empty afterward, and an Animal News paper is left on the floor where he was standing. If the cell still has an occupant, the release interaction has not gone through yet.

If the vent will not open, the cause is almost always one of three things. You are not on Shift 6 yet and the second floor is still closed, you are on the wrong side of the lab and standing over ordinary flooring, or you walked over the grate instead of staying on it and jumping until it gave way.

What else is in the room under the lab

A shelf near the cell holds Animal Corporation Incident Report #011. It records Squiggles, listed under his real name Hernald Ruperts, as a morale entertainer employed at the hospital for eighteen years. After consuming six to nine lab samples he attacked patients in Room 9, left for the surgery wing, and came back with a scalpel, stabbing six patients before staff restrained him. Corporation personnel collected him the following morning and the report was buried.

The Animal News paper he leaves behind covers two items. It notes that the Laboratory and several patient rooms have been closed and sealed with no reopening date given, and it reports a fatal collision between a passenger car and a Nom Aly Burgers delivery truck.

Once Squiggles is out, the room stops being useful for anything except the shortcut door, so treat the trip as a one-off per account. The achievement stays in your Journal, and the passage between the lab and the Reception office remains the fastest way to move between floors on later runs.
